Application for Service Connection

  • Requirements

  1. Photocopy of any valid I.D.
      • SSS
      • TIN
      • PhilHealth
      • Driver’s License
      Postal I.D.
  2.  1×1 I.D. Picture of the Applicant and Spouse
  3.  Duly Accomplished Application for Membership and Electric Service
  4.  Barangay Clearance
  5.  Latest Electric Bill of Nearest Neighbor
  6.  Pre-Membership Education Seminar (held every Thursday 9:00AM – 11:00AM, NUVELCO HQ)
  7.  Electrical Plan Duly signed by PEE
  8.  Electrical Permit from Municipal Engineer
  9.  Certificate of Final Electrical Inspection
  10.  Payment of the Following Fees Membership Fee – P100.00
      • Minimum Share Capital – P1200.00 (12 Share @ P100.00/share)
      House wiring Fees/Coop Shares
  1. Consumer applies for house Connection.
  2. House Wiring Clerk advices the consumer to submit/complete the house wiring requirements.
  3. House Wiring Clerk prepares Charge Billing for payment.
  4. Consumer pays the application fees (House Wiring, Membership Fee, etc.)
  5. House Wiring Clerk prepares the Turn-On, RV and other requirements for tapping.
  6. Consumer Section Head reviews the requirements and affix his/her signature.
  7. MSD Chief verifies the completeness of the requirements and affix the signature and forwards to the ISD Manager for approval.
  8. Internal Auditor reviews and approves the application.
  9. ISD Manager approves the requirements and application.
  10. General Manager receives the application for final approval.
  11. Warehouseman prepares the materials for release.
  12. Tapping Crew receives the materials and complete papers.
  13. Tapping Crew taps applicant as scheduled.

Change/ Transfer of Ownership

  • Initial Procedure

  • Change of Address/ Transfer of Location
  1. Fill up Application Form for Change of Ownership/Name;
  2. Photocopy of the instrument of transfer/waiver of ownership of land/building to include transfer of share capital.
  3. Pay a Transfer Fee of P 200.00 and P 100.00 Membership Fee if not yet a member;
  4. Clearance from the Area Office indicating that all obligations are paid;
  5. Fill up Application Form for Membership and pay the minimum share capital of One Thousand Two Hundred Pesos (P 1, 200.00) if not yet a member of the Cooperative;
  6. Present Valid ID e.g. SSS/GSIS, Driver’s License, Voter’s ID etc.
  7. A copy of Official Electric Bill Receipt (OEBR) of the original owner.
  1. Fill up Application Form for Change of Name;
  2. Photocopy of Death Certificate of the original/registered owner;
  3. Clearance from the Area Office;
  4. If the spouse is still living, the OEBR shall be named after him/her;
  5. If both spouses are deceased, a waiver of the surviving children shall be executed in favor of one of them as transferee to include the transfer of share capital;
  6. Pay a membership Fee of One Hundred Pesos (P 100.00) and a transfer fee of Two Hundred Pesos (P 200.00) for the share capital.
  1. Copy of OEBR;
  2. Photocopy of Valid ID (Postal ID, Driver’s License, SSS/GSIS etc.)
  1. Letter request from the owner;
  2. Clearance from Area Office;
  3. Electrical Plan if needed;
  4. Payment of fees based on inspection and assessment.

  2. House Wiring Clerk advices the consumer to submit/complete the house wiring requirements.
  3. House Wiring Clerk prepares Charge Billing for payment.
  4. Consumer pays the application fees (House Wiring, Membership Fee, etc.)
  5. House Wiring Clerk prepares the Turn-On, RV and other requirements for tapping.
